What Are Biologics? Understanding a New Frontier in Bronchiectasis Care
The word “biologic” can sound intimidating, but chances are you’ve already benefited from one.
Biologics are medicines made from living organisms or their components, such as proteins, cells, or bacteria. Unlike traditional medications that are chemically manufactured, biologics are produced using biotechnology. This allows them to target specific parts of the immune system with remarkable precision.
For many conditions, that targeted approach can improve effectiveness while reducing unwanted side effects.
You’ve Probably Already Received a Biologic
If you’ve ever received a vaccine, you’ve already experienced a biologic.
Examples include vaccines that protect against:
Influenza (flu)
Hepatitis B
Human papillomavirus (HPV)
COVID-19
These vaccines work by teaching your immune system to recognize and fight specific viruses before they cause illness.
A Personal Perspective
More than 25 years ago, I underwent a hysterectomy after being diagnosed with cervical carcinoma in situ caused by HPV. At the time, an HPV vaccine wasn’t available. Today, I’m grateful that my grandchildren can be protected against this cancer-causing virus because of advances in biologic medicine. It’s a powerful reminder of how medical research continues to improve lives across generations.
How Biologics Are Used Today
Biologics aren’t just used to prevent disease. They have transformed treatment for many chronic inflammatory and autoimmune conditions, including:
Asthma
Severe allergies
Rheumatoid arthritis
Psoriatic arthritis
Psoriasis
Atopic dermatitis (eczema)
Crohn’s disease
Ulcerative colitis
Certain cancers
Rather than suppressing the entire immune system, many biologics target specific inflammatory pathways involved in disease. Depending on the condition, they may block immune proteins such as IgE, IL-4, IL-5, IL-13, or TNF-alpha, helping reduce inflammation while leaving much of the immune system intact.
Could Biologics Help People Living with Bronchiectasis?
This is one of the most exciting areas of bronchiectasis research.
At present, no biologic is FDA-approved specifically for bronchiectasis. However, researchers are actively studying whether biologics may benefit certain groups of patients, particularly those who also have:
Severe asthma
Allergic bronchopulmonary aspergillosis (ABPA)
Eosinophilic airway inflammation
Asthma-bronchiectasis overlap syndrome (ABOS)
Early studies and case reports suggest that some patients may experience:
Fewer exacerbations
Better lung function
Improved quality of life
Reduced need for oral corticosteroids
Although these findings are encouraging, larger clinical trials are still needed to determine which patients are most likely to benefit.
Are Biologics Safe?
Like all medications, biologics can cause side effects. However, they undergo rigorous testing before receiving approval and continue to be monitored after they reach the market.
Because biologics target specific immune pathways instead of broadly suppressing the immune system, they often provide a more precise treatment option for appropriate patients.
Your healthcare provider can help determine whether the potential benefits outweigh the risks based on your individual medical history.

Biologics have already transformed medicine—from preventing infectious diseases with vaccines to treating complex inflammatory conditions that once had few effective options.
While biologics are not yet approved specifically for bronchiectasis, ongoing research offers hope that they may become an important treatment option for selected patients in the future.
