The Mucociliary Escalator: Your Lungs’ Natural Cleaning System
Have you ever wondered how healthy lungs stay clean all day long, even though we are constantly breathing in dust, pollution, allergens, bacteria, viruses, and other microscopic particles? We have a pretty remarkable built-in cleaning system called the mucociliary escalator. Although the science behind it is complex, its purpose is simple: to trap unwanted particles and germs and move them out of our airways.
How the Mucociliary Escalator Works
Healthy airways continuously produce a thin layer of mucus. We often think of mucus as something undesirable, but it actually plays an important protective role. As we breathe, mucus traps dust, air pollution, allergens, bacteria, viruses, and other inhaled particles before they can travel deeper into the lungs.
But trapping them is only half the job. We also have to get them out. Lining our airways are millions of microscopic, hair-like structures called cilia. These tiny cilia beat together in coordinated waves, moving mucus upward through the airways toward the throat. Think of it as a microscopic conveyor belt that is operating around the clock.
Once the mucus reaches the throat, we may cough it out or simply swallow it without ever realizing it. In healthy lungs, this process happens continuously in the background, helping protect us from infection and keeping our airways functioning normally.
What Happens in Bronchiectasis?
In bronchiectasis, this remarkable cleaning system does not work as efficiently. The airways have become abnormally widened and damaged; mucus can pool in these widened areas, and normal mucociliary clearance may be impaired. The cilia may not move mucus as effectively, particularly in damaged areas of the airways.
At the same time, people with bronchiectasis often have increased mucus production. So now we have a difficult combination: more mucus to clear and a less efficient system for clearing it. Instead of being steadily transported upward and out of the lungs, mucus can become trapped in the damaged airways.
Why Trapped Mucus Matters
Mucus that remains in the lungs isn’t simply uncomfortable. It can create an environment where microorganisms can persist and grow. This can contribute to infection and inflammation, which may cause additional airway damage. That additional damage can then make it even harder for the lungs to clear mucus.
And around and around we go. You may hear this described as the vicious cycle of bronchiectasis or, more recently, the vicious vortex, because several processes can occur and interact simultaneously.
Understanding this also helps explain why mucus clearance is about much more than simply trying to stop coughing. Our cough is one way our body tries to compensate for a mucociliary clearance system that isn’t working as effectively as it should.
This Is Where Airway Clearance Comes In
This is why daily airway clearance is such an important part of bronchiectasis management. I like to think of airway clearance as helping our lungs do a job they are having difficulty doing on their own. We are essentially providing some assistance to that impaired mucociliary escalator.
Depending on your individual needs, airway clearance may include exercise, huff coughing, Active Cycle of Breathing Techniques (ACBT), Autogenic Drainage, postural drainage, oscillating positive expiratory pressure (OPEP) devices such as the Aerobika®, a high-frequency chest wall oscillation (HFCWO) vest, and nebulized saline, including hypertonic saline when prescribed.
Often, several techniques are combined because they accomplish different things. Nebulized saline may help hydrate and loosen mucus. An OPEP device may help mobilize mucus and move it toward larger airways, while postural drainage uses gravity to assist mucus movement. Exercise increases ventilation and airflow, and huff coughing can help move mucus upward and out.
It isn’t necessarily about finding one perfect airway clearance tool. It is about creating a routine that helps move mucus from where it is stuck to where your body can clear it.
Airway Clearance Isn’t Only About What You Cough Up
This is an important point because we sometimes judge an airway clearance session by the amount of mucus we see afterward. But mucus doesn’t necessarily travel from the smaller airways all the way out of our mouths in one step.
Airway clearance techniques can help mobilize secretions from smaller or more peripheral airways toward larger, more central airways. From there, coughing and whatever mucociliary clearance remains can continue helping move those secretions upward. So even when you don’t produce a dramatic amount of sputum during an airway clearance session, you may still be accomplishing something important.
Moving mucus counts.
Finding the Airway Clearance Routine That Works for You
There isn’t one airway clearance routine that is appropriate for everyone with bronchiectasis. Our lungs are different. The location and extent of our bronchiectasis are different. The amount and thickness of our mucus are different. Our underlying causes, infections, reflux issues, physical abilities, and other medical conditions may also be different.
That is why bronchiectasis guidelines recommend that airway clearance techniques be taught and individualized by a respiratory professional with expertise in airway clearance. What I do for my airway clearance may not be the best or most appropriate for you.
Once you understand the techniques that are appropriate for your lungs, consistency becomes incredibly important. Airway clearance isn’t something we do because our lungs are “dirty.” We do it because one of the lungs’ normal defense mechanisms has become less effective.
And that distinction matters. Our goal is to support the lungs' natural physiology: hydrate mucus, mobilize it, move it upward, and clear it.
The mucociliary escalator may not function as efficiently in bronchiectasis, but we aren’t powerless. With the right airway clearance techniques, we can give our lungs a helping hand and support this remarkable cleaning system every day.
